Support coordinators, allied health professionals, LACs, and community organisations can refer participants directly to our team. We respond promptly and work collaboratively to ensure a smooth, person-centred transition.

Who Can Make a Referral?

We welcome referrals from a wide range of professionals and organisations who work with NDIS participants. Whether you are a support coordinator, a hospital discharge planner, a GP, or a family member acting on behalf of a participant, we are here to help.

Community Access

Supporting participants to engage with community activities, social events, and essential services.

Daily Living Support

Assistance with personal care, meal preparation, household tasks, and building everyday independence.

Supported Independent Living (SIL)

Tailored support in shared or individual accommodation settings, focused on safety and independence.

Respite Care

Short-term accommodation and care giving participants and their carers a well-deserved break.

Permanency Support Program (PSP)

Family preservation, restoration, foster and kinship care support, and case management for children and families.

What happens next

How the Referral Process Works

Step 1

Submit the Form

Complete the referral form below with the participant's details, NDIS plan information, and support needs.

Step 2

We Review Your Referral

Our intake team reviews the referral within one business day and confirms receipt by phone or email.

Step 3

Initial Consultation

We contact the participant (or their representative) to arrange an initial consultation and needs assessment.

Step 4

Service Agreement

We develop a tailored service agreement aligned with the participant's NDIS plan goals and funding.

Step 5

Supports Begin

Supports commence and we maintain regular communication with the referrer throughout the onboarding process.
